2. Eligibility Requirements ALVIA's egg donation programs are intended for prospective donors who meet applicable eligibility requirements. Preliminary eligibility criteria may include:
Being within the age range specified for the relevant donor program.
Meeting applicable physical and medical eligibility requirements.
Being in generally good physical and mental health.
Providing accurate personal and medical history.
Being willing to complete the required screening and evaluation process.
Meeting additional requirements established by participating fertility clinics.
Specific eligibility requirements may vary depending on the clinic, intended parents, applicable laws, and medical recommendations. Meeting preliminary eligibility criteria does not guarantee acceptance into an egg donation program. Final eligibility is determined through the applicable screening and approval process, including independent medical evaluation. ALVIA reserves the right to decline or discontinue an application where program requirements are not met, subject to applicable law.
3. Application Process Prospective donors may begin the process by submitting an application through the Website or another authorized ALVIA communication channel. The application and donation process may include:
Submission of an initial application.
Preliminary eligibility review.
Communication with an ALVIA representative.
Completion of a detailed donor questionnaire.
Submission of photographs, videos, and relevant background information.
Creation of a donor profile within ALVIA's internal database.
Review of the profile by prospective intended parents through participating fertility clinics.
Potential matching with intended parents.
Additional interviews, consultations, and medical screening.
Execution of applicable agreements and informed consent documents.
Medical preparation, monitoring, and egg retrieval.
Post-procedure recovery and follow-up coordination.
The sequence and duration of these stages may vary. Submission of an application does not guarantee matching, approval, or participation in an egg donation program. Submitting an application does not create an obligation to undergo egg donation. Any obligations arising after the execution of separate program agreements will be governed by those agreements and applicable law.
4. Donor Profiles and Database ALVIA maintains its own internal database of prospective egg donor profiles. These profiles may contain information voluntarily provided by applicants, including personal characteristics, educational background, photographs, videos, and relevant medical or family history. Donor profiles remain within ALVIA's database and are not routinely transferred or sent as complete profile files to fertility clinics or intended parents. Instead, authorized intended parents may be granted access to view selected donor profile information through arrangements facilitated by participating fertility clinics. Access is intended solely for legitimate donor evaluation and matching purposes. ALVIA may implement access restrictions, authentication measures, and other reasonable safeguards to protect donor information. Viewing access does not grant recipients ownership of donor profiles or permission to reproduce, distribute, publish, or otherwise use donor information outside the authorized matching process. Any separate disclosure of donor information, including information required for medical screening, legal documentation, or program administration, will be handled in accordance with applicable law, ALVIA's Privacy Policy, and any required authorizations. ALVIA does not guarantee that technical safeguards can eliminate every risk of unauthorized access, copying, or disclosure.

Donor identity and confidentiality arrangements may vary according to applicable program requirements, agreements, and laws. ALVIA does not guarantee absolute anonymity or permanent confidentiality where disclosure is required or permitted under applicable law. 7. Voluntary Participation and Informed Consent Participation in an egg donation program is voluntary. Prospective donors have the right to receive information about the proposed program and to ask questions before making decisions. Medical informed consent must be obtained by the responsible healthcare professionals in accordance with applicable laws and professional requirements. Donors may communicate concerns or questions to ALVIA and the participating medical team throughout the process.
